Getting back on your feet following surgery, a traumatic incident or the diagnosis of a serious illness could be one of the toughest challenges you may ever face. FutureCare Health operates 12 skilled nursing and rehabilitation centers across the Baltimore Washington area, providing clinical expertise.
FutureCare Health and Management Corporation has joined the nursing home quality campaign “Advancing Excellence”. The mission of Advancing Excellence is to achieve excellence in the quality of care and life of their nursing home residents. This collaborative national campaign includes leaders from long term care providers, consumers, advocates, ombudsmen and government agencies. Together they have established an infrastructure to provide support and tools to nursing homes across America.
Currently the companies facilities are participating in two of the “Advancing Excellence” campaigns. The first campaign is “Hospitalizations”. Hospitalizations goal is to prevent their residents from the trauma and risks associated with hospitalization when an acute condition can safely be managed in the facility. This goal will assist our staff to safely care for residents on-site using evidence-based and expert recommended tools and practices. This benefits their residents by receiving care from staff that know the residents’ individual preferences and needs. Residents remain in a familiar environment. They avoid unnecessary, uncomfortable trips to the hospital and potentially long delays in emergency room. Potential hospital complications will be avoided.
The second campaign is “Consistent Assignment”. Consistent assignment occurs when residents are cared for by the same caregivers particularly geriatric nursing assistants and licensed nurses. This has been shown to increase caregivers’ familiarity and relationships between the caregivers and both the residents and family members.
In conclusion, this is one of many programs that FutureCare participates in to improve their residents’ quality of life and care. Advancing Excellence provides them with tools and evidence-based expert advice on how to accomplish their goals for their residents. The two campaigns that FutureCare has chosen will help them to consistently achieve their goal of “Excellence”.

Social Worker – The Social Worker provides social services to residents, their families and significant others to facilitate coping with the impact of illness, extended care placement and/or discharge and to achieve the maximum benefit from health care services. Ā The Social Worker assures each resident is provided with the necessary care and services to attain or maintain the highest level of psychosocial well being. Ā The Social Worker works with a predominantly adult geriatric population.
